Adult Adoptions are handled through the Register in Probate Office.
Step-Parent Adoptions and Agency Adoptionsare handled through the Register in Probate Office. You may come to the office to purchase a forms packet for a fee or click on Step-Parent Adoption [PDF] or Agency Adoption Guideline [PDF] for a guideline. It may be necessary to hire an attorney to assist you with either of these procedures. You MUST contact a licensed adoption agency to complete the step-parent investigation/screening OR a Home Study Report and Recommendation of Agency. A Receipt for Medical/Genetic Information Form must be submitted to the court prior to the hearing.
Adoptions and Adoptive Placements Forms
Foreign Adoptions are handled through the Register in Probate Office. Within 365 days of a child being admitted to the United States, the Adoptive parent shall submit a letter to the Court requesting registration of the foreign adoption. A Report of Adoption form must be completed by the adoptive parents and the Clerk / Deputy and submitted to the State of Wisconsin. The Report of Adoption form can be obtained from the Register in Probate or the adoption agency.
